Based on the findings of an offsite state re-licensure survey completed on July 1, 2021, Priva Home Care was found to be in compliance with the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 51, Subpart A.

Based on the findings of an offsite state re-licensure survey completed on July 1, 2021, Priva Home Care was found not to be in compliance with the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 611, Subpart H. Home Care Agencies and Home Care Registries.

611.55(e) LICENSURE
Competency Requirements

Name - Component - 00
The competency review must occur at least once per year after initial competency is established, and more frequently when discipline or other sanction, including, for example, a verbal warning or suspension, is imposed because of a quality of care infraction.

Observations:


Based on a review of personnel files (PF) and an interview with the administrator, the agency did not conduct a competency review at least once per year after initial competency is established for seven (7) of seven (7) PFs. PF# 1, 2, 3, 4, 5, 6, and 7.

Findings include:

A review of PFs was conducted on June 28, 2021 at 12:30 PM. The date of hire (DOH) is included below.

PF#1 DOH 5/7/2020 did not contain documentation of an annual competency review for 2021.

PF#2 DOH 7/21/19 did not contain documentation of an annual competency review for 2020 and 2021.

PF#3 DOH 5/20/18 did not contain documentation of an annual competency review for 2019, 2020, 2021.

PF#4 DOH 9/15/19 did not contain documentation of an annual competency review for 2020 and 2021.

PF#5 DOH 2/9/19 did not contain documentation of an annual competency review for 2020, and 2021.

PF#6 DOH 4/8/18 did not contain documentation of an annual competency review for 2019, 2020, 2021.

PF#7 DOH 2/9/2020 did not contain documentation of an annual competency review for 2021.

An interview with the administrator on July 7, 2021 at 9:00 AM confirmed the above findings.









Plan of Correction:

Priva Home Care does and always has conducted annual trainings for all of our employees (direct care workers) to enforce competency for existing and new methods and procedures to optimally care for our clients. Annual trainings are administered online for each caregiver, where they are required to watch interactive videos and read training literature. After finishing the training, caregivers sign off with their names. A log of employees who finish their annual trainings along with when they completed them is kept on a self-populated spreadsheet by the Director. The Director is charged with making sure each employee undergoes their annual training in a timely fashion.


611.56(a) LICENSURE
Health Screening

(a) A home care agency or home care registry shall insure that each direct care worker and other office staff or contractors with direct consumer contact, prior to consumer contact, provide documentation that the individual has been screened for and is free from active mycobacterium tuberculosis.

Observations:


Based on a review of personnel files (PF), the Centers for Disease Control (CDC) guidelines, and an interview with the administrator, the agency did not conduct screening for mycobacterium tuberculosis according to guidelines for seven (7) of seven (7) PFs. PF# 1, 2, 3, 4, 5, 6, and 7.


Findings include:

The CDC guidelines state that all Health Care Workers (HCW) should received baseline tuberculosis screening upon hire, using s two-step tuberculin skin test (TST of s single blood assay for tuberculosis (TB) to test for infection with tuberculosis. After baseline testing for infection with tuberculosis, HCWs should receive TB screen annually. HCWs with a baseline positive test for tuberculosis infections should receive one chest radiograph result to exclude tuberculosis disease. CDC Guidelines for preventing the transmission of Mycobacterium tuberculosis in health care settings, 2005. Morbidity and Mortality World Report 2005;(RR-17)http://www.cdc.gov/mmwr/pdf/rr/rr5417.pdf.
*Baseline (preplacement) screening and testing, in addition to the IGRA (interferon-gamma release assay) or TST, shall include a symptom screen questionnaire and an individual TB risk assessment. Serial screening and testing not routinely recommended. Annual TB education is recommended. (CDC/MMWR/May 17, 2019/Vol. 68/No. 19).

A review of PF was conducted on June 28, 2021 at 12:30 PM. The date of hire (DOH) is indicated below:

PF#1 DOH 5/7/2020 did not contain an individual TB risk assessment and symptom screening questionnaire on hire.

PF#2 DOH 7/2/2019 did not contain an individual TB risk assessment and symptom screening questionnaire on hire.

PF#3 DOH 5/20/2018 did not contain an individual TB risk assessment and symptom screening questionnaire on hire.

PF#4 DOH 9/15/2019 did not contain an individual TB risk assessment and symptom screening questionnaire on hire.

PF#5 DOH 2/9/2019 did not contain an individual TB risk assessment and symptom screening questionnaire on hire.

PF#6 DOH 4/8/2018 did not contain an individual TB risk assessment and symptom screening questionnaire on hire.

PF#7 DOH 2/9/2020 did not contain an individual TB risk assessment and symptom screening questionnaire on hire.

An interview with the administrator on July 7, 2021 at 9:00 AM confirmed the above findings.








Plan of Correction:

In addition to the baseline TB screening that employees at Priva Home Care are required to have, The Director will now include the CDC recommended TB Risk assessment and symptom screening questionnaire in its employee application packet to be completed by a prospective employee before hire. The Director will ensure the risk assessment and symptom screening questionnaire are valid and the employee is free from any symptoms that are in question. If symptoms are present, The Director will ask the potential hire to seek professional medical advice and confirmation that TB is not present. The Director will also ensure that TB education is included in Employee Annual Training. TB risk assessment and symptom screening questionnaire will be kept in each employee's personnel file.

Based on the findings of an offsite state re-licensure survey completed on July 1, 2021, Priva Home Care was found to be in compliance with the requirements of 35 P.S. 448.809 (b).
